Why do you often see varied pagerank in different sub-directories (2nd level) of directory websites?

For example, how come a health subdirectory may have more pr than the shopping subdirectory....or maybe just the opposite sometimes?

Numerous reasons.

1) Look at the subcategories on the homepage. If the subcategory is listed on the homepage this is probably the reason why.

2) Some categories get links from other websites pointing to specific categories.

3) A directory could be new and/or that category might be new and hasn't received PR yet.

Internal linking between the pages on the website itself sometimes ranks a subpage higher than its peers.

Us directory folk know about that.

I actually had my directory fixed to show the most popular categories as subs off the main page. Prior to that my directory was set up in alphabetical order, now it makes more sense.

In addition to what has been previously posted in answer to your question, sometimes the more links you have on a sub-category page, the less the pr is. Not a rule, but it happens sometimes that a certain number of links can keep a page's pr from growing.
